Quarterly Report Requirements

The quarterly report should include details on the progress made in the current quarter, plans for the next quarter, and a detailed financial summary of the business. Items to be addressed in the report include:

•  How the PANASCI TEC award money and services benefited the business.

•  The amount of the PANASCI TEC award money spent in the current quarter and the amount to be allocated in the next quarter.

•  Summary of each item funded during the quarter and how it benefits the business.

•  Affidavits from all parties receiving funding or product (e.g. equipment) must be provided confirming receipt of funding or product and verification of how it benefits the business.

•  Any updates on the changes made to the organization (e.g. structural changes, name changes, proprietorship changes). Written documentation must support these changes.

•  Restatement the goals of this business.

•  Any written agreements on behalf of the business.

• Federal identification number must be provided in your first quarterly report.
