To apply to the Advanced CEL Program, you may download a registration form or request one by mail by contacting UB School of Management, Center for Entrepreneurial Leadership at (716) 885-5715.

Applications must be TYPED or PRINTED in black ink and suitable for photocopying. This form may be photocopied or scanned, and a photocopy may be submitted as the application. Answers must be limited to available space.

The deadline date to receive this information is August 1, 2008 .

INCOMPLETE or LATE APPLICATIONS will NOT BE CONSIDERED.
